There are 3 versions of the mount I created in FreeCad, 1 Taller for more support and 2 shorter with both the .STL and the FreeCad .FCStd files. The attached Photo's show the taller and the shorter mounted to the left, and the Shorter ones do not cover the CREALITY logo. The Left Upper one moves the mounting point for the sensor mount further to the left and optionally higher up for more clearance. When printing the STL for this file you may need to flip it upside down on the print bed so it lays flat due to the raised pad.
This mount will allow you to mix the two different mounts together for mounting the 360 run-out mount to this mount with 1 - M3 flat head 8-10mm screw fitted into the small rectangular pocket and the screw inserted from the other side.
From this point follow the directions in the video for relocating the sensor to the top. You will need an additional 3 wire extension about 12-15 inches long for the sensor. I scrounged and made mine from old harnesses I had in my junk box.
To mount the completed sensor re-locator setup, just slip it on around the Top Spool mount, hook up the sensor wires and that's it! Use the version you want, but after testing I found that in this instance, the short one with the raised pad to the left gives more room for the filament space between the print head and the sensor.
