So I want to have the PTFE closer to the hotend and made this. Kind of work in progress. Notable things:

Might need to adjust distance between PTFE guide and leveler
The gear protrudes the bottom by a tiny fraction, better leave some space for it. I have a 1mm gap so I'm fine.
The weird protrusion on the cover is a cable chain guide
Tolerance is pretty tight. Might be good to add a handle for easily removing the cover or adjust the tolerance in the future.
I ended up not putting a screw through the reduction gear. Since the bearing seems to provide enough guide and screw seems to rub the inside of the gear and make it not fit as well.
The original mounting screw is a bit short with the mount I used.
Tension is by default relatively high? Don't tension it too much, it will stop the stepper from spinning.

Installation:

Optional depending on your space/position of the base: Put a m3 hex nut in the bottom side for the bottom right screw hole. Maybe tape it so it don't fall out as well.
Install bearing on base + cover
Install the bracket first for your extruder first (not provided here)
Screw the base to the stepper with the short bottom right stepper motor screw
Would be good to screw the top left long screw in just to align and fixate the base
Assuming your hotend is installed, insert the PTFE tube
Put PTFE guide on the tube
Install the reduction gear, you might need to bend the guide a bit hard for the gear to get in
Migrate the original screw/bearing guide thingy from the original lever to the new lever
Install the lever. It's moderately tight and you will need a tiny bit of force.
Install the spring tensioner. Assemble the spring + nut/screw thing. Screw it so that it has low tension for easy installment. Put the end with spring on the lever first, then push the other side into slot.
Put the cover on. First align the PTFE guide with the slot for it. Push the cover so it click in. Then push the cover so the screw holes aligns and geos into their slots on the cover. Finally install the screws. Ideally the cover gets very close to the base, leaving a very small gap between the two.

Pretty sure it's these models but let me know if I used another variant of these. These are used so that I don't have to remodel the extruder.
