Tie a string around the buckles on the hip belt on your backback, at the top and bottom. This results in a loop of string that you place in the hooks of the Hip Belt Extension. Do this on both sides. This will allow you to use the hip belt even if you have outgrown it widthwise.

NOTE: This is not a perfect design and if you have the time and skills you should tweak this before using. (The ideal option would be to buy a modern backpack with an extra large hip belt.)

I have used this myself on a 3 day hike of about 40 km. It worked alright, but not much better than you would expect. Remember to bring a cloth or some other cushioning to put between yourself and the plastic.

I arranged mine such that the layer lines run up and down the length of the piece. This resulted in very high strength. Even though it delaminated internally, the structure as a whole is still in great shape.
