Unfortunately, the original ball head plate was of very poor quality. The thread was directly in the plastic body made of soft plastic, so it was only a matter of time before it would get damaged. 

My model includes a nut with a standard 1/4” UNC tripod thread. There isn't practically chance to damage it.

You can choose from two options. The first is with a rubber pad and the second, simpler, without a pad.

 

Print instructions:

I decided to use PETG. Printing the model does not require any special settings, supports required. See the attached 3MF file for detailed print settings.
