This is a parametric dog tag using the OpenSCAD customizer.
In case your not familiar with OpenSCAD it's a free programmatic CAD program. You write code to make the model. Don't be intimidated by the programming part because I've done that part for you. All you have to do is install OpenSCAD, open the provided file, and change values in the customizer to create the dog tag you want. When your dog tag looks like you want hit F6 to render and when that's done hit F7 to save the STL file.

The font name is not the name of a font file but the logical font name. A list of installed fonts can be obtained by selecting Help → Font List in OpenSCAD.
By changing filaments at the proper layers you can get contrast between the tag body and the name and phone number.
