Couldn't justify paying $100+ for one of these things so quickly made my own.   Made for 10mm pipe.  I used 10mm plastic coated steel tube which I had lying around which had OD of 10.2mm and 1.5mm wall thickness.   I printed at 100% infill for a bomb proof print…don't think this will ever break.

Drill a hole about 1 cm from the end (not like I did) to allow room for a knot if needed.    

The section where the rope winds on you need at least 170mm long from hole to bend to give enough room for the rope. i.e. around 120mm for rope to wind on.  The pin that goes into the extension uses the same pipe as handle but really it should be 12mm diameter.  It worked just fine with 10mm though.  Held in place with a 50mm M3 nut and bolt but you could just glue it in (I figure being steel it might rust and need replacing in a few years so having a bolt secure it should make life easier in future)

Tested with KA and Severne Race sails that require a lot of down haul with 3 and 4 pulleys and worked perfect.   Put so much downhaul on I thought the rope would break.

I could add handle that spins etc but really not needed so better to keep it simple.
