Version 1 (red print in photos) I created based on a similar design I found, but I made mine based on the type of siding I have, dutch lap. I printed it with a 4mm nozzle and 20% infill out of PLA. The plate is 3/8" thick. The triangle part that juts out is about ½" deep and is 1 ¼" in height. The spacing was very specific to fit the hole/wiring from my old doorbell.

Version 2 (white in the photos) I modified to make it work better with the optional angle bracket that came with the doorbell. The middle hole is offset and the recesses are oblong to match that bracket. I also made the plate ¼" thick as I thought 3/8" might be excessive. I printed with the same settings, this time with silver PLA, and I spray painted it white to match the doorbell and add some durability. This thinner version should probably have been printed with a higher density because it flexed if I tightened the screws too much. Also, the siding seemed to buckle more with this version, creating a slight gap between the siding and the back of the plate. Using finely threaded screws helped pull the siding somewhat closer, but I'm not sure how else to resolve this, besides perhaps adding some sort of thin insulating material to fill the void. I did put some silicone caulk in the old doorbell holes.
