This is a case for the Password Pump V2, Rotary Encoder Model, created by Dan Murphy. The kit, and the fully assembled unit, is available on Tindie at the Password Pump Store (https://www.tindie.com/stores/passwordpump/). The one pictured is mine. There is a saying: "If you can remember all your passwords, you are probably doing it wrong."
It's a little fiddly to attach the standoffs inside the case, so I used an M2 screw and nut to hold the standoff for assembly – see the pictures. I managed to crack the corner of a display before I figured that out.
This was previously posted on Thingiverse, and I moved it here due to their increasingly user-unfriendly practices.
This is Rev 2 update c. Let me know if you have any issues with it, I will attempt to fix it.
