I created the model using the Kiri Engine photogrammetry app for iPhone, with final editing and repairs made in TinkerCad.
The scan didn't capture much lettering detail, so I cut black vinyl letters with my Cricut machine. The lettering SVG file is included.
I used grey PLA to print, then painted with Behr Hammered Brown Gloss spray paint. After I applied the lettering, I sprayed the piece with clear acrylic. Finally, I added some small brass screws.
Printed with .20mm layer height; no supports (it's a solid model). Any infill will work, 20% (or even a little less) is fine.
This was my test print, so I printed it small. If I do another, I'll increase the size to cover the full print bed.
Shout out to @frankattack, who did this identical project with different software. Our results are startlingly similar:

Frank mentions that he may try to scan the plaque again for higher quality. My fingers are crossed!!
