Print instructions

Print the lenses with the thread on top.

Use a little negative "horizontal expansion" (cura setting, no idea if available in other slicers) for easygoing threads. I used -0.1 for ultimaker2. Ultimaker 3/5 will need a bit more, no idea about the tolerances of other brands.
