I hesitated to upload this because it is fairly specific in its application, but this is a dust boot that I designed for a Shapeoko 3 CnC router with a Makita router.  I have been using it for several years, and the  only required maintenance is to occasionally replace the shroud, which is made from a clear 2 liter cola bottle.

The router is has a 65mm diameter, and my vacuum hose is 2.5 inch (~60mm).

It does require a little additional hardware:

¼"x20 bolt

¼"x20 wing nut

¼" washer

4 M3 screws

clear 2 liter cola bottle

2 sided tape

After attaching the two halves I cut a couple of 2 inch strips from the bottle.  I attach two sided tape to the flat surface around the ridge, then attach the clear plastic strip to that.  It takes one full strip, then a piece of the second.

I start the first strip at the edge of the split, and cut the second at the other edge of the split.

After attaching, I cut slits up to the plastic between ½" and ¾" to make fingers.  Over time the fingers will bend and tear, but with a good vacuum system the boot will still collect dust well.

Once it starts looking really bad, I just peel off the shroud and used tape, and make another.

The boot in the photo is several years old and still looks like new.
