Geek Pi apparently sells two different versions of this heat sink, that look very similar. One version has screws into the post-holes on RasPi, and the other that apparently just sticks on with thermal tape. The stick-on version has two wires to connect with the Pi (red & black), while the screw-in version has 3 wires (red, black & blue).
This case appears to be for the version that just sticks on, which was the wrong version for my heat sink. I ended up having to cut the posts out of two of the corners to make it fit.
My print ended up being kind of janky but the model certainly works.
